Clinical Question: In veterans with post-traumatic stress disorder (PTSD), does quetiapine monotherapy compared to placebo improve symptoms with tolerable adverse reactions? Search Strategy: A PubMed search was conducted to locate primary literature on the use of quetiapine as monotherapy in veterans with PTSD. Search terms used were ââ¬Å"quetiapineâ⬠, ââ¬Å"monotherapyâ⬠and ââ¬Å"PTSDâ⬠to ensure appropriate studies were retrieved by the search. Rationale for Selection of Literature: The literature selected must addresses the use of quetiapine as monotherapy for PTSD. Thus, studies of quetiapine as adjunctive therapy were ruled out as they were irrelevant to the clinical question. After reviewing the available data from the literature search, theâ⬠¦show more contentâ⬠¦There was a 1 week single-blind lead-in with placebo where non-responders (n= 80) were randomized in 1:1 ratio to quetiapine or placebo treatment for 12 weeks (1). Initially, patients started at a dose of 25mg at bedtime and clinical response and tolerability were used to guide titration (1). At the end, the dose ranged from 50 to 800mg daily with an average of 258mg daily for the quetiapine arm (1). Other psychotropic medications may not be used during the study except rescue mediations during the first 2 weeks including chloral hydrae for insomnia or agitation and lorazepam for anxiety or agitation (1). Participants were followed up during week 1, 2, 4, 8 and 12 for evaluation (1). The pri mary outcome was the total Clinician-Administered PTSD Scale (CAPS) score which the quetiapine arm had a greater reduction of score (75.40 to 53.85) than placebo arm (70.60 to 65.66) after 12 weeks with p=0.02 (1). For secondary outcomes, quetiapine arm also had higher reduction in CAPS re-experiencing score, CAPS hyperarousal score, Davidson Trauma Scale and Hamilton Depression Rating Scale (HAM-D) scores than the placebo arm at the end (1). However, there was no difference in CAPS avoidance/numbing score and negative symptom subscale of the Positive and Negative Syndrome Scale (1).
